Premier Medical Group is committed to making every patient’s experience as pleasant as possible. You can avoid any unnecessary inconvenience or frustration by following the general recommendations below.

What to Bring to Your Appointment

  • Insurance card
  • Physician referral forms if required by insurance
  • List of current prescription or over-the-counter medications you are taking, including dose and frequency
  • Pertinent information about your medical and surgical history
  • Recent x-rays or appropriate records you may have

Visitor Policy

Premier Medical Group is taking all necessary measures and precautions to protect the safety of our patients and staff.

We thoroughly clean and sanitize all patient waiting rooms, exam rooms, and other care areas according to the recommendations of the Centers for Disease Control and Prevention (CDC).

All staff, patients, and visitors are also asked to adhere to social distancing guideline and you may notice increased spacing of seating in common waiting areas.

To limit the overall traffic in the practice, we are limiting patients to one (1) essential visitor/companion, and asking patients to not bring any visitors, unless absolutely necessary.

Patients, staff and visitors at Premier Medical Group offices are required to wear a face mask at all times. This measure has put in place for your protection, as well as the protection of patients and staff. We ask for your understanding and appreciate your cooperation in our commitment to keeping our community safe.

 

Insurance and Payment Information

Premier Medical Group is a provider for Medicare and most major insurance plans. We provide insurance billing. Any services not covered by insurance will be the responsibility of the patient.

For those services not covered by insurance, Premier Medical Group requests payment at the time of appointment. Your insurance company may also require you to pay a co-payment at the time of your appointment. When necessary, our staff will work closely with those patients who require a payment plan.
