- Ejaculate directly into specimen container.
- Replace lid and label container with your first and last name, date of birth, and collection time.
- Specimen CANNOT be more than 1 hour old!
- Protect specimen from extreme temperature changes during transport.
- Please make sure you have provided all billing info to the staff when dropping off specimen.
- Call Premier Medical Group at the number above for all inquiries regarding test results approximately 5 business days after specimen has been dropped off at lab.
- We will need 1 negative test results (no sperm seen).
- Please wait at least 1 week in between sample collection.
SPECIMEN SHOULD BE BROUGHT TO EITHER LABCORP OR QUEST LABS DEPENDING ON YOUR INSURANCE. PLEASE MAKE SURE THAT YOU KNOW WHICH LAB PARTICIPATES WITH YOUR INSURANCE BEFORE BRINGING SAMPLE OTHERWISE YOU MAY BE RESPONSIBLE FOR PAYING FOR THIS TEST.
