Our multidisciplinary team of specialists provide leading treatment options for our male fertility patients. Physicians may recommend one or more of the procedures performed at The Male Fertility Center at Premier.

Microsurgical Varicocelectomy

Microsurgical varicocelectomy is a treatment option for varicoceles, a condition that results in severe pain due to enlarged scrotal veins. Microsurgical varicocelectomy reduces pain in the scrotum and can also improve fertility in cases where varicoceles have negatively impacted testosterone production. Our board certified urologists are trained to carefully determine eligibility for this procedure, which may last for about 30 minutes to an hour depending on whether or not enlarged scrotal veins are present in one or both of the patient’s scrotums. Full activity can resume after 2-3 weeks of recovery time.

Microsurgical Vasectomy Reversal

This surgical procedure is designed to undo vasectomies, enabling patients to regain fertility. During the procedure, the physician reconnects the vas deferens (a vasovasostomy), the tubes that allow sperm to enter the semen. At times, the vas deferens needs to be reconnected to the epididymis (a vasoepididymostomy). Once a vasectomy reversal is successfully completed, a patient may then be able to fulfill desires for growing his family. Learn more about vasectomy reversal.

Scalpel-free Vasectomy

Scalpel-free vasectomy is a minimally invasive procedure that enables patients to prevent unwanted pregnancies by disconnecting the vas deferens. During this procedure, the urologist makes a small puncture which is then stretched to a small degree, allowing the doctor to see and disconnect the vas deferens.

Microsurgical Sperm Extraction

Sperm extraction is recommended for patients who have difficulty producing or releasing healthy sperm. In such cases, our team may perform microsurgical sperm extractions from the testicle (TESE), the epididymis (MESA), or both. Through this procedure our multidisciplinary team focuses on obtaining the best quality and quantity of sperm necessary to fertilize an egg, leading to pregnancy. We also focus on minimizing damage to the reproductive organs so as to help ensure future fertility and other pregnancies. The patient may also choose to cryopreserve sperm during this procedure for future IVF/ICSI.

Hormone Replacement

Our male fertility physicians work closely with patients to determine the best approach for those in need of hormone replacement. Since testosterone, when it comes from outside of the body, can suppress the production of sperm, our team will evaluate a patient’s fertility goals before recommending the use of testosterone or hormone replacement.

Treatments for Chronic Scrotal Pain

Chronic scrotal pain, also referred to as chronic orchialgia or testicular pain syndrome, affects the testicle, epididymis and spermatic cord. This is usually diagnosed in cases where pain in this region of a patient’s body lasts for more than three months, interfering with a patient’s quality of life. If medical therapy fails, a spermatic cord block is performed in the office; local anesthesia is injected in the area that houses the nerves to the scrotum. If this is partially or completely successful, treatment for chronic scrotal pain may include microdenervation of the spermatic cord.

Microsurgical Spermatic Cord Denervation

Microsurgical spermatic cord denervation is a treatment option available to patients who have been diagnosed with chronic scrotal pain. This procedure typically takes about 45 minutes and involves the use of general anesthesia. Patients who undergo the procedure can expect to return to normal activity in as little as 1 to 3 days after the procedure. As the name suggests, microsurgical spermatic cord denervation reduces the sensitivity of nerves within the vas deferens and the spermatic cord. This, in essence, prevents the nerves from sending a signal of pain from the testes to the brain.
